For 70 years fluoride toothpaste was the only mainstream choice for cavity prevention. In the past decade hydroxyapatite (HAp) toothpaste has emerged as a credible alternative, with several head-to-head studies showing comparable effectiveness in cavity prevention without the controversies that surround fluoride for some patients. Here is what the evidence actually shows.
How fluoride works
Fluoride incorporates into tooth enamel, forming fluorapatite — slightly stronger and more acid-resistant than the natural hydroxyapatite that makes up enamel. Decades of research support fluoride toothpaste at 1000-1500 ppm for cavity prevention in most populations. Best evidence is for school-age children and high-caries-risk adults.
How hydroxyapatite works
HAp is the same mineral that enamel is made of. Synthetic nano-HAp particles bind to enamel surface and effectively remineralize early lesions. Studies from Japan (where it has been used since the 1980s) and recent European trials show comparable cavity prevention to fluoride at 10% concentration.
Head-to-head evidence
Several randomized trials have compared HAp to fluoride toothpaste over 12-18 months. The conclusions: HAp is roughly equivalent for cavity prevention in moderate-risk adults. For high-risk patients (history of cavities, dry mouth, frequent decay), fluoride still has stronger evidence base.
Who should pick fluoride
Children 6+ with developing dentition (research is most extensive). Adults with active or recent cavities. People with dry mouth (Sjogrens syndrome, certain medications). Anyone who has tolerated fluoride well historically. The safety profile at toothpaste concentrations is excellent.
Who should pick hydroxyapatite
Adults with sensitive teeth (HAp specifically helps with sensitivity). Pregnant women (some prefer to avoid fluoride during pregnancy as a precaution; evidence does not require this). People with thyroid concerns (fluoride competes with iodine at very high doses; not a concern at toothpaste levels but some patients prefer caution). Anyone who has had adverse reactions to fluoride.
What about combining them
Some products contain both. Reasonable for high-caries-risk adults who want the dual mechanism. Not necessary for most people. Brushing technique and time spent matter more than the specific active ingredient.
Pricing reality
Fluoride toothpaste runs $3-8 per tube. HAp toothpaste typically $10-20 per tube. The cost difference reflects manufacturing scale, not effectiveness. For most adults, fluoride at $4 produces equivalent results to HAp at $15.

Is fluoride safe?
At toothpaste concentrations (1000-1500 ppm) and used as directed, yes. The toxicity concerns refer to chronic high-dose exposure (severe overdose or industrial exposure), not normal use.
Does HAp work as well for kids?
Less evidence than for adults. Pediatric dentists still primarily recommend fluoride for children due to the depth of evidence.
Should I switch to HAp if I have fluorosis?
Yes, reasonable. HAp does not contribute to fluorosis and provides similar protection.
Can I use whitening toothpaste with HAp?
