Clear aligner orthodontic treatment has become enormously popular. What is less commonly discussed is the specific gum health considerations that come with clear aligner therapy and what happens when gum health is not maintained during treatment.
Why aligners create specific gum health challenges
The removable nature of clear aligners is the main feature that makes them preferable for oral hygiene compared to fixed braces. You can remove them to brush and floss normally. The challenge is that many aligner wearers do not use that advantage optimally. Not cleaning teeth and aligner trays before reinserting after eating creates an environment where plaque accumulates at the gum line under the aligner with the trapper effect of holding food particles and bacteria in sustained contact with gum tissue and enamel surfaces. Aligner attachments create additional plaque retention sites that require extra care during cleaning.
Gum changes that can occur during treatment
Gingivitis is common during clear aligner treatment particularly if hygiene habits do not adapt to the demands of the treatment. With adequate hygiene gingivitis during orthodontic treatment is largely preventable. Some patients develop gum recession during clear aligner treatment. This can be treatment-related where tooth movement outpaces the supporting bone or hygiene-related where gum disease was either pre-existing or developed during treatment. Pre-treatment assessment of gum health and bone levels is important. Starting orthodontic treatment with existing untreated periodontal disease significantly increases recession risk.
How to maintain gum health during aligner treatment
Remove aligners before every meal and snack. Brush and floss before reinserting. Clean aligners with a soft brush and mild soap not toothpaste which is abrasive and clouds the plastic. If you eat or drink anything other than water with aligners in rinse and brush immediately before reinserting. Attend all scheduled check-up appointments where the dentist can monitor gum tissue health alongside treatment progress.
