Dental implants have become the gold standard for tooth replacement — superior to bridges (which require grinding adjacent teeth) and removable dentures for most patients in most situations. The research on long-term outcomes is extensive and largely favorable, but important nuances around who does well and what maintenance is required deserve attention before committing.
Success rates: what the data shows
The most commonly cited implant success rate in systematic reviews is around 95% over 10 years for well-placed implants in appropriate patients. Long-term data now follows some patients 20+ years — cumulative survival rates at 20 years in favorable patient populations run around 86-90%. These are outcomes that exceed most prosthetic devices in medicine.
What predicts success or failure
Smoking. The single strongest modifiable risk factor. Smokers have roughly 2-2.5 times the implant failure rate of non-smokers. The same wound-healing impairment and immune suppression that worsens gum disease dramatically affects osseointegration (the fusion of implant to bone) and increases peri-implantitis risk. Many implant surgeons strongly counsel quitting smoking before implant placement.
Diabetes. Poorly controlled diabetes significantly increases failure risk. Well-controlled diabetes (HbA1c under 7%) is associated with outcomes approaching those of non-diabetics. The same mechanisms that worsen periodontal disease impair osseointegration and peri-implant tissue health.
Bone quality and quantity. Implants require sufficient bone volume and density. Areas with bone loss from periodontal disease or from tooth loss itself may require bone grafting before or concurrent with implant placement.
Peri-implant health maintenance. Peri-implantitis — infection of bone and tissue around an implant — is the most common cause of late implant failure. It’s essentially periodontal disease affecting implant sites, and it’s more difficult to treat than natural tooth periodontitis because implant surfaces don’t have the same biological attachment. Prevention through meticulous hygiene and regular professional maintenance is far more effective than treatment.
Oral health and implant success
History of severe periodontal disease is a recognized risk factor — the same susceptibility that led to original tooth loss can predispose to peri-implantitis around implants. This doesn’t mean implants aren’t appropriate for people who’ve had periodontal disease — many do well — but post-placement maintenance is even more important.
