Loose teeth in adults are a completely different matter from loose baby teeth. Adult tooth looseness indicates that something significant has damaged or is destroying the support structures of the affected teeth. It does not happen from aging alone.
The dominant cause: periodontitis
Advanced periodontal disease is the most common reason for adult tooth looseness. As periodontitis progresses the bone that anchors the teeth is gradually destroyed. When enough bone is lost the tooth begins to have measurably increased mobility. At early stages of bone loss this is not perceptible to the patient. As it advances the tooth begins to feel loose and may shift in position. Addressing the underlying periodontitis with professional treatment and sustained maintenance can stop further progression but bone that has been destroyed does not regenerate with routine treatment. Stabilization rather than reversal is the goal.
Trauma
A direct blow to the face can loosen a tooth by partially or completely disrupting the periodontal ligament. Acute trauma to teeth requires prompt dental evaluation. Knocked-out permanent teeth can sometimes be successfully reimplanted if treated within 30 to 60 minutes of the injury.
Bruxism
Severe chronic bruxism can cause tooth looseness through direct trauma from excessive occlusal forces and secondary bone remodeling in response to those forces. Bruxism-related looseness responds to occlusal treatment such as night guards and bite adjustments. We cover bruxism in detail in our bruxism article.
What can be done
The treatment depends entirely on the cause and severity. Periodontitis-related looseness requires treating the periodontitis. Trauma-related looseness is managed based on severity from monitoring to splinting to root canal therapy. Bruxism-related looseness responds to night guard use. The sooner loose teeth are evaluated and the cause addressed the more options remain available. A loose permanent tooth should always be evaluated by a dentist promptly.
