Navigating it means understanding what the different options actually do, how the results compare, and what the relevant risks are.
How whitening works
Whether you are using OTC whitening strips or a professional in-office system the active ingredient is hydrogen peroxide or carbamide peroxide which releases hydrogen peroxide. The peroxide penetrates the enamel and oxidizes the organic chromogen molecules embedded within the tooth structure. The differences between products are primarily concentration and contact time.
OTC whitening strips
OTC strips available in the US typically contain 6 to 10 percent hydrogen peroxide. They are worn for 30 to 60 minutes daily for 1 to 2 weeks. Multiple clinical trials confirm significant whitening effects. Results tend to last several months before touch-up is needed. Cost: 30 to 60 dollars for a complete treatment course. They may cause sensitivity that limits use for some people and do not always reach gum line areas as consistently as custom trays.
Take-home professional trays
Custom-fitted silicone trays made by a dentist filled with 10 to 22 percent carbamide or hydrogen peroxide gel. The custom fit allows consistent gel contact across all tooth surfaces including interproximal areas the gum line regions that strips may not cover well. Custom trays can be reloaded with gel for touch-up whitening months or years later. Cost: typically 300 to 600 dollars for the initial trays and gel. Results are generally superior to OTC strips for completeness and evenness of coverage.
In-office whitening
High-concentration hydrogen peroxide gel of 25 to 40 percent applied by a dental professional with gum tissue protection. The main advantage is speed with dramatic whitening in a single 60 to 90 minute appointment. The main limitations: higher sensitivity risk, results may continue to change for several days after treatment, and cost of 500 to 1,500 dollars. Take-home tray whitening achieves similar final results at lower cost with less sensitivity if the patient is willing to wait a few weeks.
